Semaglutide 12-Month Supply

TERMS AND CONDITIONS OF SEMAGLUTIDE SUBSCRIPTION SERVICE

1. ACCEPTANCE OF TERMS
Welcome to Allure Medical! These Terms and Conditions of Service ("Terms") govern your use of our Semaglutide or weight loss injections program (the "Service"). By enrolling in the Service, you accept and agree to be bound by these Terms.

2. SUBSCRIPTION PERIOD AND RENEWAL
The Service is a 12-month subscription program. Your subscription begins on the date of enrollment and will automatically renew for additional 12-month periods unless you provide written notice of non-renewal at least 30 days prior to the expiration of the then-current term. No early cancellation is permitted during a subscription period.

3. PAYMENT TERMS
The total program fee is $2028.00, payable in 12 equal monthly installments of $169.00 each. Payments will be automatically charged to the credit card you provide at the time of enrollment on a recurring basis. You agree to keep a valid credit card on file at all times during the subscription period. Delinquent payments may incur a $50.00 late fee.
Prepayment Option: If you elect to prepay the discounted 12-month subscription program fee of $1859.00 at the time of enrollment, you will have received the equivalent of one month of Service at no additional charge.

4. NO CANCELLATION
You acknowledge that the Service is a 12-month commitment, and no cancellations or refunds will be issued during the subscription period. Monthly payments are non-refundable, even if you fail to use the Service or comply with the program.

5. HEALTH REQUIREMENTS
The Service is only suitable for individuals who meet the health criteria provided by Allure Medical. You represent that you have reviewed these criteria and are eligible to participate in the program. You must consult with a healthcare provider before starting the program.
 
6. MEDICAL DISCLAIMER
The Service is not intended to diagnose, treat, cure, or prevent any disease. Results may vary and are not guaranteed. You acknowledge that weight loss depends on many factors, including diet, exercise, and overall health.

7. INDEMNIFICATION
You agree to indemnify and hold harmless, its officers, directors, employees, agents, and affiliates from any and all third-party claims, liability, damages, and/or costs (including but not limited to attorney fees) arising from your use of the Service.

8. GOVERNING LAW
These Terms shall be governed by and construed in accordance with the laws of the state of Michigan, without giving effect to any principles of conflicts of law.

9. AMENDMENTS
Allure Medical reserves the right to modify these Terms at any time. Your continued use of the Service constitutes acceptance of any changes.
